Endocrinology & Metabolism

Goal & Educational Purpose The resident should develop the skills to recognize, diagnose treat endocrinologic and metabolic problems that will be seen in general internal medicine practice; the resident should know when to refer cases to a more specialized physician or center; extensive skills should be developed for diabetes mellitus, thyroid problems, hypertension, lipid disorders and menopausal management.
Objectives
  • Diagnoses and management of all aspects of diabetes mellitus – Type 1 and 2, peripheral manifestations and complications, referral to ophthalmologist and surgeon, DKA, H HNK, hypoglycemia, insulin resistance, insulin regimens, oral agents, operative and preoperative cases, physiologic problems, pregnancy, adolescent diabetes, etc.
  • Same for thyroid disorders – hypo, hyper types, nodules, neoplasia, goiters, masked types, operative management, recognition and emergency treatment of thyroid storm, etc.
  • Hypertension – skill in suspicion, diagnosis and treatment of secondary and essential types, extensive knowledge of most recent JNC report.
  • Lipid problem evaluation and treatment with extensive knowledge of most recent NCEP report.
  • Osteoporosis and management of post-menopausal female, osteomalacia.
  • Causes of and management of hyponatremia
  • Causes of and management of hypercalcemia and hypocalcemia, diagnosis of hyper and hypo – parathyroidism.
  • Clinical signs and symptoms of acromegaly and other growth hormone disorders of adults, some exposure to growth disorders of children.
  • Clinical signs and symptoms of hypopituitary and excess pituitary hormone disorders.
  • Diagnoses, treatment, general management, operative management of hypoadrenal and hyperadrenal states.
  • Clinical signs, symptoms and differential diagnosis of pheochromocytoma
  • Appreciation of complexities, presentations and occult nature of porphyria
  • Recognition and diagnosis of hemachromatosis
  • Acquaintance with hypo- and hyper-testosterone state
  • Knowledge of and skills to evaluate impotence in men.
Educational Content
  • Read and understand JNC the most recent guidelines
  • Read and understand the most recent NCEP guidelines
  • Read MKSAP  endocrine and metabolism section and answer questions
  • Read endocrine and metabolism section of medicine textbook
  • Consult endocrine and metabolism specialty text
  • Rotate with endocrinologist for 1 month performing consultations, in-patient evaluations and working extensively with outpatients
  • Observe Diabetes Center management of patients
  • Attend Camp  No-Koma if on that month
  • Attend 8:00 AM Endocrine conferences all year
  • Attend 12:00 noon conferences given by S. Grubb MD all year
Evaluation
  • Resident will evaluate rotation and attending
  • Attending will evaluate the resident
  • ITE endocrine section results
  • ABIM certifying results for endocrine section
  • Reports from residents meetings
