Nephrology

Goal & Educational Purpose The resident should gain competency in the diagnosis, management and evaluation of patients with renal disease, genitourinary problems, and hypertension that are managed by a general internist with and without the input of nephrologists and urologists.
Objectives
  • Assessment of acute changes in renal function – acute renal failure and acute changes in chronic renal failure; differentiation of pre-renal, renal and post renal deterioration by history, physician examination and laboratory; and when to use consultant.
  • Emergency management of acute renal failure that may include dialysis but to include emergency management of life threatening problems to include hyperkalemia, calcium abnormalities, etc.
  • Continuing management of chronic renal failure patients pre and post dialysis and transplantation
  • Indications for renal biopsy
  • Assessment of renovascular disease
  • Indications and use of special tests, such as renal scan, IVP, captopril test, creatinine clearance, urologic studies (cystogram, retrograde studies), etc.
  • Hypertension – measurement, diagnosis, treatment, select groups, compliance
  • Upper and lower urinary tract infections, abscesses, UTI associated sepsis, urethritis
  • Renal tubular acidoses, tubulo-interstitial disease, drug related renal failure
  • Nephrolithiasis
  • Immune disease of kidney – lupus, scleroderma, HUS, etc.
  • Diabetes mellitus and the kidney
  • Fluid management, electrolyte management
  • Expertise in urinalysis
Educational Content
  • One month rotation with nephrologist seeing consultations and evaluating inpatients
  • Renal clinic
  • Office practice with nephrologist
  • Dialysis unit observation and evaluation of patients
  • Placement of temporary hemodialysis lines and peritoneal dialysis catheters with permission and under direction of nephrologist
  • Observe renal biopsy
  • Review urinalysis with nephrologist and/or laboratory personnel
  • Review pathology specimens with nephrologist and/or pathologists
  • Attend nephrology 8:00 lectures throughout the year
  • Review imaging studies with nephrologist and/or radiologist
Evaluation
  • Resident will evaluate attending and rotation
  • Attending will evaluate resident
  • Results on renal section of ITE
  • Results on renal section of ABIM certifying examination
