Aging adults with serious disabilities face difficult decisions about how and where they will live in ways that are consistent with their wishes. Their predicament is that increased dependence upon long-term care by others makes them vulnerable to loss of control over key decisions in their lives, significant lifestyle changes, and overwhelming financial costs. The most drastic outcome is nursing home placement although home-based care may pose similar risks.

Those who fare best in making transitions of this kind have trusted family, friends, neighbors, church caregivers or others upon whom they can rely for enduring personal support and advocacy. Person-Centered Planning provides a method to organize this support to assure they remain empowered in key decisions affecting their lives and long-term care.

Through this eight event workshop series, providers of health care and social services can learn to engage elders in creating circles of support for creative problem-solving, assess risks to independent decision-making and care, and identify challenges to coping with significant transitions.

Learning activities will engage participants in mini-presentations with group discussions. Training manuals with guidelines and additional resources are provided. An optional computer-based on-line learning module will be available for extended study and feedback.
