PROMISSORY NOTES

Promissory notes for all Direct and university loans can be completed on-line at:
Direct Loans: http://dlenote.ed.gov
Perkins, Health Professions, Nursing, Disadvantaged, Primary Care or WV Medical Student and Institutional Loans: http://www.ecsi.net/prom31

Direct loan, Perkins, Health Professions, Nursing, Disadvantaged, Primary Care and WV Medical Student loan notes are considered Master Promissory Notes (MPN) which means you will not have to sign a note for any subsequent years unless you have repaid the loan in full or no disbursement has been made in a 12 month period.

Institutional loans require a co-signor. Complete your information on-line and then print the promissory note and give it to your co-signor to complete. Return the promissory note to West Virginia University, PO Box 6003, Morgantown, WV 26506. This is not a master promissory note so you will have to sign a new promissory note each year you are awarded. It is important that Admissions & Records have current addresses for you.  Contact them at (304) 293-2124 or go to the STAR system to verify, and if necessary, update your addresses.

You must complete each promissory note legibly and accurately according to the instructions that will be provided.  Because a promissory note is a legal document, it must be completed precisely.
