Social Worker

The social worker who works as a member of a healthcare team views the consumer holistically by helping the consumer meet their biopsychosocial needs. Working directly with the consumer and his/her family, the medical social worker provides a variety of services which include but are not limited to discharge planning (preparing the home, the consumer and the family for discharge), brief counseling, locating necessary resources such as social security or insurance, contacting local private and non-profit services to meet needs, helping the family and the consumer to understand the nature of the care needed at discharge, and providing emotional support and communication of resources. Social workers are compassionate, multi-tasking professionals who advocates for policy change, assess consumer needs, and communicate consumer needs with healthcare team and vice versa that enhances the healthcare experience.
