Office Reminder Materials

• Chart cards attach to the left inside cover of patient medical records and provide space to record all cancer screening data.
• Patient reminder cards fold to protect confidential information and include space to inform patients of the need to schedule an appointment
• Fluorescent chart stickers remind clinicians of the date of the patients last Pap test and mammogram
