What to bring with you:
- Insurance card (s)
- Prescription card
- Emergency contact information, this must be different then your contact information
- Current medication list
- List of questions for doctor or nurse (see some examples here)
- A copy of your advance directive/medical power of attorney (first appointment only).
The Referral Office schedules your “new patient” or first appointment. The MBRCC clinic does have an automated appointment reminder system that calls you 24-48 hours before a scheduled appointment. If you need to cancel or reschedule your first appointment, please call the
Referral Office directly at 304-598-4552. Please be aware that calls from us may not be recognizable on caller ID.
To make or change an appointment
At your first appointment, you will be asked to fill out a health assessment form. This form tells us about your medical and surgical history, as well as other important information we need to create your plan of care.
If you are referred to us by another doctor, or if you referred yourself, please be sure, we receive all necessary medical records, radiology films and pathology slides from any outside facilities at least (3) business days before your appointment so our doctors have time to review this information.
We may reschedule your appointment for another date, if we do not receive the needed information on time. You can contact the Referral Office directly at 304-598-4552, before coming to confirm we have all your records. The fax number for the Referral office is 304-598-4553.
Reminders for Appointments:
- Please come 15 minutes before a doctor appointments
- Visitors are limited to no more then two in exam rooms. Other visitors are welcome to wait in the lobby.
- We may ask visitors to step out of exam rooms during physical examinations or procedures. Your cooperation is appreciated
- It is a patient’s responsibility to help the team keep track of needed prescriptions. Don’t forget to ask your doctor for new prescriptions or prescription refills before you leave the clinic. If you forget and call the Triage RN for a prescription, it can take two business days to call prescriptions into your pharmacy.
- Schedule return appointments before you leave the clinic.
