Submission of annual Assignment of Wages form for WVUH, UHA employees required

Benefit-eligible WVU Medicine employees who wish to keep using their ID badge for purchases in the Friends Gift Shop, cafeterias and Medical Center Pharmacy are required to complete a new Assignment of Wages Form and turn it in to the Payroll Department. Employees have from Wednesday, Nov. 2, until Wednesday, Nov. 30, to submit the form.

The Payroll Department is located in room 4A20, on the fourth floor of Ruby Memorial, and is open from 7:30 a.m. to 4:30 p.m. Monday through Friday.

Forms will also be accepted outside the Ruby Cafeteria from 11 a.m. to 1 p.m. Wednesday, Nov. 2, and Thursday, Nov. 3.

For weekend or off-shift employees who cannot attend during one of the scheduled times, call 304-598-4088 to make arrangements to have your assignment of wages re-enrolled.

Payroll administrators for off-site locations only should contact the Payroll Department to schedule a day and time for re-enrollment.

Forms must be notarized by a notary and cannot be mailed to the Payroll Department.

Employees who do not renew by Nov. 30 will have their accounts deactivated effective Jan. 3, 2017, and will not be available for purchases. No exceptions.
