Biochemistry and Molecular Biology

Forms

This lists the various forms you will need for completion of your degree. These forms are available from Claire Noel in the Health Sciences Graduate Programs Office (HSGPO) or you can click here to download the forms (Download Forms Here). We recommend that you keep a copy of each form for your personal file. Since we do not admit students directly into a MS program, unless otherwise stated the deadlines for MS are the same as for the Ph.D. program. All forms must be typed. See the Graduate Coordinator to make arrangements for this. Do not wait until the last possible minute to do these forms.

Committee Approval Form

When you need it: Prior to the second semester of your second year. This deadline is for our program and differs from the deadline on the form. What it is: The list of faculty that you will have as your advisory committee and their signatures. For the Ph.D. program, the committee will be three members of the Department of Biochemistry and Molecular Pharmcology, at least one faculty member from outside the department, and the fifth faculty member can be from either within or outside of the department. For the MS degree only 4 members are required with one from outside the department. The make-up of the committee must be discussed with your advisor. In all cases the committee chair is the student’s faculty advisor. Who gets it: Once you have the committee signatures, make a copy for yourself and return the form to the HSGPO. The signatures of the HSGPO and the Dean will be obtained for you. Changes in the committee: To change a committee member after this form is approved simply requires that you submit a memo to the HSGPO prior to the next meeting of the committee. This memo should be signed by the committee chair. All committee members must be present at the final defense. If a substitute becomes necessary at this point, the request must be made in writing to the HSGPO and be signed by the committee chair, the student, and both the original faculty member and the substitute faculty member. Only one substitute is allowed and they must represent the same academic discipline or specialization.

Plan of Study Form

When you need it: After 18 hours of coursework for MS students and after 30 hours of coursework (do not include Bioc. 496 research credits) for Ph.D. students. For Biochemistry students this is generally after the second semester of the second year. What it is: This form lists all the courses the student has taken, the degree you are applying for and the title of your dissertation. It requires the signature of all your committee members. For students readmitted into the Ph.D. after completing a MS, course work used as part of the plan of study for the MS can be repeated on the plan of study form for the Ph.D. Who gets it: The HSGPO, who will send a copy to the Graduate Coordinator to be placed in your permanent file in the Department.

Advisory Committee Evaluation Form - Download

When you need it: After every meeting of your advisory committee beginning with the defense of your proposition What it is: This form is a specific requirement of the Department of Biochemistry. It is completed by your advisor after each committee meeting and it is the students responsibility to have it signed by all committee members.  This form is a record of your progress during your course of study. Who gets it: The original is given to the Graduate Coordinator who places it in your permanent file. A copy is given to the HSGPO and keep a copy for yourself.

Qualifying Examination Form

When you need it: At the end of the second year after you have completed your oral exam. Note this may be different from the deadline required by the HSGPO, which is after 27 hours of course work. For MS students this is completed after the written exams, generally at the first committee meeting. What it is: This is the document that certifies that you have passed both your oral and written exams. It requires the signature of all your committee members. Who gets it: Take the original to the HSGPO and give a copy to the Graduate Coordinator.

DURING YOUR LAST SEMESTER AT WVU:

Application for Graduation and Diploma

When you need it: During the first 2 weeks of the semester in which you plan to graduate. Note: you must be a registered student in order to graduate. What it is: Indicates your intention to graduate and allows you to indicate how you wish your name to appear on your diploma. Requires a $30 fee. If the degree is not actually earned during that semester, the student must notify the HSGPO and submit a new Application when registering for the term in which completion is again anticipated. A second fee will not be necessary. Who gets it: The HSGPO

Shuttle Sheet Request Form

When you need it: 3 weeks prior to your defense, either as a MS or Ph.D. What it is: This forms indicates that you plan to graduate soon. Upon receipt of this form, the HSGPO will check your records to be sure you have met all the requirements for graduation except for your final defense. In addition, the HSGPO will arrange to have your defense seminar posted in the Mountaineer Spirit. Posting of the defense prior to its occurrence is required by the Graduate Council. Once again, all committee members must sign this form. Who gets it: The HSGPO

Shuttle Sheet

When you need it: Prior to your defense What it is: This form proves you defended your thesis or dissertation and is signed by your committee. Who gets it: Within 24 hours after your defense you must return this to the HSGPO. Note: For students who have completed a MS degree and are continuing on for a Ph.D., you will need to complete all these forms again. The timing of the completion of some forms will differ. Check with the HSGPO to define deadlines for your particular circumstances.
