CMEDSA CONFERENCE SPONSORSHIP FORM

Please include the following when explaining your interest:

1. Explanation of Interest; why you want to go the conference in terms of professional and intellectual development, how the conference ties into your unique career focus;
   

2. Background; why you may deserve to go, i.e., your past service to the department and involvement in public health or else, if off-campus, volunteer opportunities in your community;
   

3. Return on Investment; how you will give back from the conferences, e.g., give a presentation or summarize what you learned and submit it online on the CMEDSA web site to share with other students; and
   

4. Agree that you will volunteer 8 hours for CMEDSA community service projects such as Safety Bug, the Health Fair or organizing flood relief or on behalf of CMEDSA in your community.
