Gaetano Monteleone, M.D. Dr. Guy Monteleone has extensive experience in the area of Sports Medicine, and is double board certified in both Sports Medicine & Family Medicine. Prior to becoming the Director of the Clark K. Sleeth Department of Family Medicine Sports Medicine Division, he was the Co-Director of the Sports Medicine fellowship and team physician for Wake Forest University in North Carolina. Guy has also served as the assistant team physician for the University of Pittsburgh, the NFL Cleveland Browns, and the NBA Cleveland Cavaliers. Jennifer Michael If you would like to be seen in the Sports Medicine Center you should obtain a referral from your primary care physician. Upon receipt of this referral, take it to Jen at the checkout desk and she will contact your insurance company for authorization. After we recieve authorization you will recieve a call to set up the appointment.

Family Medicine Scheduling Department

Current patients of the Department of Family Medicine can call our scheduling department at 304-598-6900, Monday through Friday 7:00 a.m. to 4:30 p.m. New patients can also be seen in the Sports Medicine Center, although, some insurance providers require a prior authorization from your primary care physician. Every attempt will be made to arrange an appointment in the Sports Medicine Center and obtain the necessary insurance authorizations prior to your appointment.
