Division of Occupational Therapy Dear Participant, Thank you for your interest in participating in my research study. This survey is research which is being conducted in partial fulfillment of the requirements for a master's degree in Occupational Therapy. The purpose of the research is to determine the relationship between exercise training, competitive athletic participation volume, and quality-of-life outcomes in wheelchair-bound persons with spinal cord injury. Your honest answers will help me as a future health care professional to better understand your needs. Participation will include filling out a survey of questions which should take approximately 10-20 minutes to complete. Your responses to the survey will be kept completely anonymous and confidential. You do not have to answer any question with which you are uncomfortable.
