38-year old man with jaundice, intense pruritus and dark urine

Gross Description
At autopsy, the liver was slightly enlarged and weighed 2,200 gm. The external surface of the liver was smooth and discolored dark green (figure 3). On cut section, a 3.5 cm, firm whitish tumor mass was present in the perihilar region around the main intrahepatic bile ducts with an infiltrating margin, more around the right duct. Multiple, similar satellite nodules were present on cut sections. In addition, multiple, soft, bile-stained foci with diameters ranging from 0.5 to 1.5 cm were present on the cut surface. The remaining liver parenchyma was dark green and firm, and appeared fibrotic. The lymph nodes in the porta hepatis contained firm white areas.
