Case of the Month September 2003

A 43 year-old man with cough, hemoptysis and shortness of breath

Jason Fowler, PA; Michael Szwerc, MD and Scott Nestor, DO

Radiographic Findings

Intravenous contrast enhanced CT imaging through the chest demonstrated eccentric and somewhat lobular soft-tissue thickening of the trachea located just proximal to the carina and extending 4-5cm superiorly (Figs 1-2). This lesion was found to cause significant occlusion of the airway at the level of the aortic arch. No other lesions were identified within the lungs, or elsewhere. These findings, in conjunction with the patient’s history of hemoptysis, weight loss and stridor suggested a malignant process; therefore, an outpatient bronchoscopy and biopsy were ordered.
