Case of the Month January 2003

A 82-Year-Old Female with a History of Postmenopausal Bleeding

GROSS DESCRIPTION 

The right ovarian cystic mass measured 9.5 x 9.0 x 5.2 cm and weighed 210 g. It was grey to yellow-tan with necrotic areas and invaded through the capsule in areas. The cyst was 6.5 cm in size and its inner surface was irregular with tumor nodules. The left ovary measured 3.5 x 3.0 x 2.3 cm and contained a 3.2 cystic tumor, which was grey-tan without necrosis or hemorrhage. There were multiple tumor nodules up to 2.5 cm in greatest dimension on the omentum, the serosa of the small intestine and the spleen. The tumor had extensively involved the serosa/adventitia of the rectosigmoid colon, and in one area the tumor extended into the colonic wall without involvement of the mucosa. The lateral portions of the uterus and the posterior aspect of the cervix were also extensively involved by the tumor. No obvious tumor was present within the uterine cavity. The 90g spleen contained a 2.8-cm pale to yellow-tan tumor mass.
