Case of the Month November 2005

A 35-year-od female with a right pneumothorax

Macroscopic Examination

Right Lung; pneumonectomy:

The right lung measured 23cm x 16cm x 14cm. The right pulmonary artery was small in size. There were focal areas of soft tan necrosis and exudate on the pleural surface. Sectioning demonstrated multiple area of infarction ranging in size from 4.3cm x 3.9cm to 8.0cm x 6.5cm. Multiple nodules, some with central necrosis, ranging in size from 0.9cm to 1.4cm and multifocal areas of necrosis were demonstrated throughout all three lobes. The middle lobe bronchioles contained soft tan purulent material. No discrete masses, cavities, or fibrosis were identified. Seven perihilar and peribronchial lymph nodes ranging in size from 0.8cm to 1.1cm were dissected and were grossly unremarkable.
