Case of the Month December 2005
Two Patients with a History of Thromobosis
Tiffany Harper MD; Patricia Canfield MD
Patient History
Patient #1
A 25 year old male presented to the physician’s office with a history of left lower leg pain. On physical examination, the patient’s leg was slightly tender to palpation with a 1 cm difference in size of the left leg from the right (the left leg was larger). The patient had a venous doppler ultrasound examination performed which demonstrated superficial thrombophlebitis. The following thrombophilia panel of laboratory tests was performed: protein C, protein S, antithrombin III, factor V Leiden, prothrombin G20210A mutation and lupus anticoagulant.
Patient #2
A 60 year-old female in good physical health was referred for a thrombotic workup because of a positive family history. Her son had a history of multiple episodes of deep venous thrombosis, and her daughter had a prior history of multiple miscarriages. The patient’s remote history was significant for superficial thrombophlebitis and venous insufficiency involving her lower extremities, but she had no history of deep venous thrombosis in spite of her heavy smoking and long term hormonal therapy. |
