Case of the Month March 2006

A 28-year-old woman with a left cerebellar hemispheric lesion

Imaging Data

An MR scan of the brain revealed a non-enhancing posterior fossa lesion involving the left cerebellar hemisphere and a portion of the vermis.  The lesion was hyperintense on T2-weighted images (Figure 1) and measured approximately 7.9 x 7.7 x 7.0 cm.  Areas of cystic change were noted throughout.  There was mass effect with displacement of the fourth ventricle toward the right.  Rostral and caudal herniation of brain tissue was noted through the tentorium and foramen magnum, respectively.  No signs of hydrocephalus were present.  A non-enhancing superior cervical cord syrinx was also noted.
