A 20-year-old with Irregular Menses
Nathaniel Bailey, MD, Jim Williams MD
Patient History
A 20-year-old female presents to her gynecologist complaining of irregular menses for approximately one year. Additionally, she reports that she recently becomes full with less food than she used to, but she believes that she is gaining weight. Otherwise, her past medical history and family history are unremarkable. Upon examination, the patient has a large palpable abdominal mass. Ultrasound examination reveals an approximately 20cm in diameter left ovarian cyst.
The patient had an exploratory laparotomy and left oophorectomy.
A 1300 g, 18 cm in diameter tan cystic ovary is received for evaluation. The cyst wall is predominantly thin and smooth; however, there are multiple brown nodules on the inner surface, up to 1 cm in greatest dimension.
