Chronic exposure to toxic agents such as silica can result in severe inflammation and pulmonary fibrosis. Various lung diseases are also associated with increased incidence in fungal or tubercular infections due to weakened host defense mechanism. With recent resurgence of tuberculosis, mycobacterial infection is a serious health concern. The respiratory tract also permits delivery of drugs by inhalation. Therapeutic agents may be targeted to the upper and distal airways for localized effects, or to the alveoli for local or systemic action. Because of the large surface areas and the relatively low content of digestive enzymes in the alveoli, the pulmonary route can be a viable alternative to the gastrointestinal tract for the delivery of peptides and proteins, which, with recent advances in biotechnology, have become the drugs of choice in contemporary drug therapy. Advancement in drug delivery strategies and in treatment of pulmonary diseases requires a clear understanding of the host defense mechanism against external stimuli. This involves multiple biochemical events manifested by the oxidative/proteolytic responses of alveolar macrophages (AM) and epithelial cells, cytokine-mediated cell-cell interactions, and the alteration of receptor-mediated processes in cell uptake or secretion of bioactive material. It is from this emphasis that Dr. Ma originates his current research. A) Pulmonary Fibrosis and the Search for Antifibrotic Agents Ma and co-workers have made a major accomplishment in establishing tetrandrine as the first effective antifibrotic agent against silicosis through cellular-based mechanism, targeted drug delivery, and in vivo treatment studies. The mechanisms of fibrosis and drug action jointly identified the involvement of AM-orchestrated inflammatory sequel in silicosis. Tetrandrine, a bisbenzylisoquinoline alkaloid, binds to the cytoskeletal proteins of viable AM, inhibits silica-induced secretion of oxygen radicals and proinflammatory cytokines (IL-1 and TNF-a), and prevents cytokine-mediated neutrophil infiltration and fibroblast production of lung collagen. Our current interests: 1) Cell-Specific Inflammatory Response: This research addresses two hypotheses regarding the inflammatory process: oxygen radicals (OR) as second messengers and direct inflammatory mediation by type II cells. Studies have shown that OR may serve as second messengers in AM expression of neutrophil chemoattractants. Our interest is to examine of the induction of intracellular OR by fibrotic and nonfibrotic stimuli, the role of OR in NF-kB activation, and cytokine production in AM and in type II cells. Although AM play a major role in pulmonary host defense, type II cells are directly involved in the epithelial injury and repair. The oxidative and cytokine responses of type II cells toward exogenous exposure may in fact be the first step of stimulant-lung interaction. --- Cytokines are the powerful local protein mediators that are of major importance in virtually every biological process. In Dr. Ma=s laboratories, research are conducted to elucidate the cutting-edge information on the involvement of cytokines in pulmonary fibrosis and in cell activation associated with epithelial permeability and protease activity. --- 2) Mechanism of action of Tetrandrine and Derivatives for antifibrogenic effects. B) Fundamentals in Pulmonary Drug Targeting We have demonstrated that microemulsions and receptor binding liposomes (mannose, transferrin) can be used to enhance drug delivery to lung tissue or to the intended cell types. In addition, our research interests also include: 1) Cell-Derived Liposomes: This research examines the interaction of liposomes with specific lung cells based on surface molecule-mediated cell-cell recognition. Lymphocytes, for example, may be made into liposomes with surface molecules recognizable by AM, as part of the phagocytic clearance. 2) Tumor Cell-Derived Targeting System: Tumor cells, depending on their cell surface properties, exhibit specific affinity toward the capillary endothelium in their metastatic organ. Drug loaded B16-F10 tumor cells (DLTC) have been shown in our laboratory to retain selective affinity toward lung tissue, and such affinity is diminished by extensive trypsinization. Continuing research will address several questions regarding the molecular-based affinities among tumor cells and between tumor cell and metastatic tissue, the immune response and organ clearance of dead tumor cells, and the utility of DLTC for the prevention and treatment of lung metastasis. C) Transport and Degradation of Peptides Across Alveolar Epithelium Pulmonary type II cell-derived cell monolayer has been used as an epithelial model for peptide transport studies. The usefulness of this model may hinge on our understanding of cell function changes resulting from the transformation of type II to type I cells. Our current interest: 1) Characterization of the Alveolar Epithelial Model: Cell transformation from type II to type I may result in lost of certain protease activity and changes in cytokine production. These cell types may also show different responses to external stimulation on cell activities. This research examines the differentiation between type I and type II cells in metalloproteases (e.g. EP 24.11 and 24.15) activities and in type II-derived cytokines using intact, sonicated, and PMA-primed cells, appropriate peptides such as luteinizing hormone releasing hormone (LHRH) as substrates, and protease inhibitors. 2) Role of AM in Peptide Degradation: AM are known for their protease activities and their capacity to respond to a wide variety of stimuli in oxygen burst and phgocytosis, suggesting that AM may play a major role in peptide degradation in the lung. This research studies the kinetics and inhibition of AM protease activity under control and stimulated conditions and the localization of cytosol and matrix metalloproteases (MMPs). --- The MMPs are a family of enzymes that catalyze the degradation of extracellular matrix macromolecules. There is considerable evidence that individual MMPs are elevated in inflammatory diseases, such as fibrosis. They are also involved in the release of a wide range of biological factors including cytokines, receptors, other enzymes, and adhesion molecules from the cell membrane. --- 3) Peptide Absorption Studies: This research examines a number of epithelial models combining AM and type I/II cells for peptide permeability and enzyme degradation studies.
