Antibotics Greater Than Fourteen Days Supply Criteria |

WVBMS allows a maximum days supply of fourteen days and one refill on antibiotic prescriptions.
Claims for antibiotics with a greater than 14 day supply will require prior approval.
Some exemptions:
Low dose antibiotics for urinary tract prophylaxis.Each request is reviewed with current literature to determine its appropriateness.
Anti-tuberculosis agents
Anti-viral agents for HIV
