Prefilled Insulin Syringe Criteria 

Criteria for Prefilled Insulin Syringes

These items are considered convenience items by the WVBMS unless one of the following criteria are met:

  1. Patient has sufficient visual difficulties to prevent them from self administering the insulin.
  2. Patient has a physical disability that would not allow the proper administration of insulin.
  3. Patient is a juvenile patient performing self-administration of insulin in an educational situation
