Beneficiary Designations

Basic Life Insurance:
Change form |
This is the
Life Insurance policy that UHA provides on your behalf. The policy is
for Two (2) times your annual base salary and is provided automatically for
benefits eligible employees. |
Supplemental Life Insurance:
Change form |
This is
optional additional coverage that you may choose to purchase on
yourself, your spouse, and your eligible dependents. As the policy
holder, you are automatically beneficiary for anyone you are covering under
your policy. The designation is for the event that you should pass
away. |
Retirement Plans:
|
The
retirement plans through UHA are subject to ERISA law. If you are
married, (whether separated or living together) the ERISA law requires that
your spouse be listed as Primary Beneficiary for a minimum of 50%. If
you should wish to designate your spouse for less than 50% as a Primary
beneficiary, then your spouse MUST SIGN CONSENT and that consent must
be notarized. |
