**If you have previously declined coverage and now need to add the coverage please keep in mind that The Guardian has a "Late Entrant" policy.  This policy states that penalty wait periods will be implemented for those who declined coverage upon initial eligibility to the plan.  The penalty wait periods will be 6-months for Basic Services and 12-months for Major Services.  Click here to review the Plan Document
