Flexible Spending Accounts
The Flexible Spending Accounts requires re-enrollment each year for participation.
If you wish to participate for the plan year 2010 (1/1/10 - 12/31/10) you MUST complete an enrollment form and submit it to UHA Human Resources PRIOR to the close of the Benefits Annual Enrollment Period (12/15/09).
