POSITION SUMMARY:    Assess and treat individuals with mental illnesses or substance abuse problems in office and telehealth settings.  Such services include individual and group therapy, outreach, crisis intervention, social rehabilitation and teaching skills for everyday living for patients of all ages (children, adolescents, adult and geriatric issues) in an integrated outpatient primary care and behavioral health practice.. 

 

Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable.  Such service may include, but are not limited to.   

 

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

Master’s Degree in related field

2.

Licensing or certification at the following levels:

Licensed to practice in the state of Maryland

Licensed Clinical Social Work (LCSW-C), OR

LCPC-C .

 

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1.

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

2.

Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, family and/or group therapies as well as grief and bereavement work.

3.

Screen, identify, diagnose, treat and manage mental health and/or substance abuse problems in patients and family members. Provide training to other health care professionals in the same manner. May perform these functions independently or as part of a team.

4.

Assist with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, domestic violence, rape, etc. Provide training to other health care professionals in the same manner. May participate in on-call or after hour’s coverage. Provide consultative services to health care team members within scope of care definitions as needed. Maintain a working knowledge of relevant medical/legal issues that affect patient care, e.g., advance directives, child and elder abuse.

5.

Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.

6.

Participate in multi-disciplinary health care teams and provide leadership in representing clinical social work perspective/liaison with patient and family.

7.

Provide teaching and training for students, staff, and faculty from social work and other disciplines. Contribute to the academic mission of the medical center and health system by participating in academic activities such as committees, conferences, publications, teaching and research.

8.

Maintain working knowledge of and liaison with community agencies and resources.

9.

Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

10.

Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.

11.

Attend and participate in staff, committee, department, and other administrative meetings.

12.

Keep current with social work and health care developments and seek to increase further enhancement of job related knowledge.
