POSITION SUMMARY:  The Social Worker comprehensively plans for the coordination of care for the WVU Medicine patient population across the continuum. Performs resource management provides patient psychosocial support and referrals to community based organizations. Works collaboratively with the multidisciplinary care team to facilitate achievement of desired treatment outcomes. The social worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance from a variety of special funds and agencies. In addition, carries a caseload as the primary care coordinator and may offer crisis intervention to patients and families with psychosocial needs.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

 Masters Social Work Degree.

2.

 LGSW OR LCSW OR LICSW certification in the state of West Virginia.

 

 

PREFERRED QUALIFICATIONS:

EXPERIENCE:

1.

One to three years of experience preferred.

1.

Provides psychosocial assessments of patients and families to identify emotional, social, and environments strengths and problems related to their diagnosis, illness, treatment and/or life situation.

2.

Collaborates with all members of the multidisciplinary team to facilitate appropriate community resource assistance for designated caseload.

3.

Monitors the patient’s progress; intervening as necessary and appropriate, to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective.

4.

Provides education to patients and families around issues related to adaptation to the patient’s diagnosis, illness, treatment and/or life situation.

5.

Meets directly with the patient and/or family to assess needs and develop an individualized care plan and to arrange community resources to meet the individual’s needs in collaboration with the patient’s care team.

6.

Formulates, develops, and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, family and/or group therapies as well as grief and bereavement work.

7.

Works with the multidisciplinary team to assist with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence, rape, etc. Provides consultative services to health care team members within scope of care definitions, as needed. Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse

8.

Screens, identifies, diagnoses, treats and manages mental health and/or substance abuse problems in patients and family members.

9.

Documents assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.

10.

Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

11.

Maintains a working knowledge of community agencies and resources and serves as a liaison between them and the healthcare system.

12.

Collaborates for appropriate resource and financial management which may include but is not limited to-financial assistance coordination/referrals, entitlement program coordination/referrals, or patient benefit coordination.

13.

Keeps current with social work and health care developments and seeks to increase further enhancements of job related knowledge.

14.

Provides teaching and training for students, staff, and faculty from social work and other disciplines. Contribute to the academic mission of the medical center and health system by participating in academic activity, such as committees, conferences, publications, teaching and research.

15.

Exhibits professional behavior on a consistent basis.

16.

Attends and participates in staff, committee, department, and other administrative meetings.
