POSITION SUMMARY:  The Social Worker provides clinical mental health services; including, assessment, crisis intervention, ongoing therapy, and referrals as appropriate. The Social Worker assesses and refers patients for supportive services.  Provides counseling and education for obstetrics and gynecology patients  virtually and  in the outpatient clinics of WVU Medicine.

Provides assistance and guidance to individuals experiencing medial and/or psychosocial crisis precipitated by their diagnosis, or subsequent developments. Performs resource management, facilitation, and referral to other levels of care. The Social Worker intervenes with patients who have complex psychosocial needs, who require referral assistance for social programs and funding sources. . . The social worker serves as patient advocate and counselor, performs psychological assessments, refer patients and families to medical resources and provide patient and family assistance and  assists with referral when appropriate.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

Master’s Degree in Social Work.

2.

 Must have LICSW and appropriate licensure for the state of practice.

1.

Provide assessment, , ongoing counseling, and crisis intervention to patients and their families in relation to social, psychological, financial, and family situations

2.

On basis of preliminary risk screening, assesses patients and family’s psychosocial risk factors through clinical evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to diagnosis, , and ability to cope. The Social Worker will also be expected to participate in the ongoing management of those patient’s needs as deemed appropriate.

3.

Documents interventions according to departmental policies and procedure related to the interactions with patients and families emotional, social, and financial consequences of illness and/or disability access and mobilizes family/community resources to meet identified needs.

4.

Collaborates with all member of the Multidisciplinary Team. Builds and maintains relationships with members and healthcare providers to obtain input and updates when developing an individualized plan of care.

5.

Meets directly with patient/family to assess needs and develop an individualized care plan in collaboration with physician, and continues to monitor ongoing management of that patient/family.

6.

Promotes, coordinates, and refers to community education programs regarding WVU Medicine services.

 Provide education as needed to staff, physicians, and patients for transitional planning needs.

8.

Coordinates supportive counseling services and develops appropriate patient support group systems, and makes appropriate referrals.

9.

Assesses patient information and serves as advocate.

10.

Makes financial referrals for all indigent patients, as needed.

11.

Maintains patient advocacy and ensures services are rendered satisfactorily.

12.

Improve patients and their families’ knowledge regarding the importance and components of quality health care.

13.

Advocate for the primary needs of the family, including food, shelter, safety, crisis intervention, transportation and child care.

14.

Facilitate appropriate referrals and follow-up procedures to acquire necessary resources for patients.

15.

Evaluate activities and environment of clinic to enhance services to  patients.

16.

Interacts with community agencies, staff and members to enhance services to patients.

17.

Maintains professional competency to assure delivery of quality patient service.

18.

Attends educational activities to assure current in-depth social work knowledge and licensure.

19.

Participates in planning and implementing change and maintaining and improving productivity through attendance and participation in staff meetings, committees, task forces, projects, and discussions within WVU Medicine.

20.

Use Data to drive decisions and plan/implement performance improvement strategies related to assigned patients, including fiscal, clinical and patient satisfaction data.

21.

Serves as a preceptor for medical staff and any other learners as deemed appropriate.
