POSITION SUMMARY:  Assess and treat individuals with mental illnesses or substance abuse problems. Such services include: individual and group therapy, outreach, crisis intervention, social rehabilitation, and teaching skills for everyday living for patients of all ages, (children, adolescents, adult and geriatric issues) in a behavioral health environment.

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

 Master’s Degree in Social Work or Counseling.

2.

Licensed Graduate Social Worker (LGSW) OR Licensed Clinical Social Worker (LCSW) OR Provisionally Licensed Professional Counselor.

 

 

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

 Independently Licensed Social Worker (LICSW) OR License Professional Counselor (LPC).

 

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1.

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

2.

Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, family and/or group therapies as well as grief and bereavement work.

3.

Screen, identify, diagnose, treat and manage mental health and/or substance abuse problems in patients and family members.

4.

Provide psychotherapy to individuals with a variety of presenting problems including substance abuse, depression, anxiety, thought disorders, trauma, and autism spectrum. May perform these functions independently or as part of a team.

5.

Provide consultative services to health care team members within scope of care definitions as needed.

6.

Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse.

7.

Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.

8.

Participate in multi-disciplinary health care teams and represent clinical social work perspective/liaison with patient and family.

9.

Maintain working knowledge of and liaison with community agencies and resources.

10.

Maintain knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

11.

Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.

12.

Maintain electronic medical records and documentation in accordance with department, hospital, medical center and/or health system policies.

13.

Attend and participate in staff, committee, department, and other administrative meetings.

14.

Participate in department, hospital, medical center, and/or health system continuous quality improvement efforts.

15.

Participate in regular professional conferences.
