POSITION SUMMARYThis position will combine and utilize professional skills and expertise in the area of  case management of patient care in the outpatient clinic providing support for the physicians, other therapists and staff to be able to serve their patients in an efficent manner.  This position will be  responsible for handling crisis that arise for patients daily and interfacing between the patients, families and providers within the department.

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

Bachelor’s Degree in Social Work or related field required.

2.

West Virginia state and Federal criminal background check required for DHHR WV CARES regulated area

EXPERIENCE:

1.

One year of experience in Social Work required OR one year of on the job supervision at USC.

 

 

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1.

Current West Virginia Social Work licensure preferred.

 

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.  They are not intended to be constructed as an all-inclusive list of all responsibilities and duties.  Other duties may be assigned.

1.

Facilitates referrals of patients to providers.

2.

Takes crisis calls and works outcomes for patients.

3.

Sends refill requests to providers and call pharmacy per direction of MD/NP/PA as needed.

4.

Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse.

5.

Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation.

6.

Participate in multi-disciplinary health care teams.

7.

Maintain working knowledge of and liaison with community agencies and resources.

8.

Arrange, procure, and coordinate patient/family pre and post hospital/program needs.

9.

Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

10.

Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.

11.

Assisting in obtaining prior authorizations from third party carriers.

12.

Working with the units in securing beds for admission.

13.

Preparing or assisting in the preparation of discharge letters.

14.

Maintaining and updating group listings.

15.

Works with discharge planners and consult team to secure appointments.

16.

Schedules new patient visits per established protocols.

17.

Meets with patients face to face as needed to resolve crisis situations.

18.

Covers messages and work ques for providers that are out of the office.

The work environment includes time in the office at United Summit Center as well as walking around interacting with patients and staff of United Summit Center

As part of the Department of Behavioral Medicine and Psychiatry you will also interact and work with trainees of multiple disciplines.

SKILLS & ABILILTIES:

1.

Must be knowledgeable and able to access different outpatient and community support systems to grant patients access to different types of services, ie patient assistance programs, Medicaid, and welfare services.

2.

Must have the ability to be able to work closely with patients, families, physicians, psychologists, social workers and other members of the treatment team to develop individual and departmental treatment goals.

3.

Must be knowledgeable and able to access many different types of services.

4.

Must have the ability to assess and treat individuals with behavioral health and mental illnesses.
