WV Center for End-of-Life Care staff offering free help with advance directives

To recognize National Healthcare Decisions Day at West Virginia University and in Morgantown, the WV Center for End-of-Life Care will be available to assist employees and the public in completing their advance directives free of charge.

Advance directives in the form of a Living Will or Medical Power of Attorney allow individuals to make their medical treatment and end-of-life care wishes known to their family and healthcare providers.

Staff of the WV Center for End-of-Life Care will be available from 11 a.m. to 2 p.m. Friday, April 15, or by appointment in their offices in the WVU Health Sciences Center, 1195 Health Sciences North. Notaries will be present.

Advance directive forms will be available or can be downloaded at http://wvendoflife.org/for-patients/forms-and-resources for review in advance.
